Otoplasty is a type of cosmetic surgery that is used to alter the appearance of a person's external ears. This type of ear surgery is performed in order to bring the ears closer to the head. Other reconstructive procedures deal with the deformed, overly large, or abscent-microtic ears. There are two problems with the protuberant, "lop ear". The first, and most common, is that the natural crease, the "antihelical fold", that returns the outer ear direction back towards the head is abscent or under-developed. The second is that the "conchal bowl" is overly-deep and redundant and it pushes the entire ear away from the head. Otoplasty can be performed using a general anesthetic as an inpatient procedure (which is generally the case for children), but is often performed as an outpatient procedure with a local anesthetic. The procedure generally takes no more than two hours. There are two ways of performing the otoplasty. The "Open technique" surgery usually involves an incision on the backside of the ear as an approach to reshaping the ear cartilages and allowing the ear to rest closer to the head. Through that incision, the goals of recessing the conchal bowl and creating an antihelical fold can be accomplished. The Open technique procedure typically requires the patient to wear a compressive head dressing followed by a headband to allow for healing of the incisions, undermined skin, shaped cartilage, and retention-stitched areas. After 3-6 days, the compressive dressing is removed and replaced with a sweat band. Stitches are usually removed about 6 days after surgery, although dissolvable stitches may also be used. During the last decade, an "Incisionless Otoplasty" has been in use that allows for remodeling of the ear cartilages and creation of an antihelical fold without an incision. Due to the lack of incisions and skin undermining, the complications related to the Open approach are avoided. There is therefore no need for post-operative compression bandages and retention headbands.
